Overview
MAPflow Incident reporting includes all mandatory data fields and core functionalities required by OCP. In partnership with ISMP Canada, MAPflow utilizes the Medication Safety Self- Assessment (MSSA) to fulfill the Safety Self-Assessment requirement. We have designed our platform in consultation with both ISMP Canada and OCP to balance the goals of the AIMS program (minimizing preventable harm) with ease of use for busy pharmacy professionals.
How to Report an Incident
- From the Dashboard, select Incidents from the left-hand menu, then select Reports.
- Select New Incident Report.
- Complete all the required fields. Select Save and Next to navigate to the next page when applicable.
- Select Submit, then Submit again to automatically submit the incident report. Note: The incident report will be marked as completed and submitted to the registry. Responses cannot be changed after submission.
- If desired, use the printer icon or download icon in the top right hand corner to print and/or save the Incident Report.
