MAPflow streamlines follow-ups and automates documentation. Our patient-directed follow-up feature is an innovative tool designed to save your pharmacy team substantial time while improving patient response rates. Available to all per-pharmacy subscriptions, this feature automates the follow-up process while maintaining quality patient care.
How to Conduct a Follow-Up (Pharmacy Professional Directed)
- From the Dashboard, navigate to Assessments Without Follow-Up.
- Locate the patient using the search bar.
- Under the Actions column, select the Start follow-up icon.
- Complete the follow-up assessment, then select Save and Next to complete the follow-up.
How to Conduct a Patient-Directed Follow-Up
Note: If you have selected "Patient Led Follow-Up" during the initial assessment, then MAPflow will automatically send an SMS/email to the patient at the appropriate time. The steps below are only required if you:
- Did not select "Patient Led Follow-Up" as the method of Follow-Up
- Want to invite your patient to start the follow-up immediately
- From the Dashboard, navigate to Assessments Without Follow-Up.
- Locate the patient using the search bar.
- Under the Actions column, select the Invite patient to start a follow-up icon.
- Select Email or SMS, enter the patient's contact information, and select the patient's preferred language
- Select Send.
Note: Selecting the Save to patient profile checkbox automatically adds any inputted patient contact information to their patient profile in MAPflow.
Once these simple steps are completed, MAPflow automatically and securely reaches out to your patient with follow-up questions. When patients receive their follow-up invitation:
- They verify their identity through a secure process.
- They answer user-friendly follow-up questions about their condition.
- Their responses are securely transmitted back to your pharmacy system.
MAPflow's system intelligently handles patient responses based on the information provided:
When symptoms have resolved:
- The system automatically documents the successful outcome.
- The assessment is tagged as "Service Complete" in your dashboard.
- The patient is removed from your follow-up list.
When issues are detected:
- The system sends an instant notification to the pharmacist.
- The patient appears under "Follow-Up Review Required" in your dashboard.
- You can easily view their status and responses.
- You can determine appropriate next steps based on the information provided.
